The "Printable Entry Form" Link below can be used in two ways. First, you may just click on the link.
Your browser should open the page showing the entry form. Just click on the "Print" button twice, to make
two copies of the form, and then use the "Back" Button to return to the website. If this does not work with
your browser, then point your mouse at the "Printable Entry Form" link, and click the right mouse button.
A menu will appear, and you will go to "Save Target As" and click with the left mouse button.
A save window will come up, and you should save the document in your "My Documents" folder. You may then
open the file with whichever word processing program you have. If neither of these methods works, please
e-mail the address below, and we will get an application to you some other way. |
